Paeonia lactiflora, commonly known as the Chinese white peony, has a long history of use in East Asian traditional medicine. In the Chinese Pharmacopoeia (2020) the dried root is prescribed as a decoction for menstrual pain, dysmenorrhea, and to calm the uterus; the same root is also macerated in alcohol to produce a tincture used for chronic pain and as a topical poultice for bruises and sprains. Korean herbalists, as documented in Kim et al. (2018), prepare a decoction of 10 g of the root steeped in 200 ml of water for 30 minutes to treat menstrual cramps and to relieve muscle tension. In Japan, the root is used in a 1:5 ethanol tincture (10 g root in 50 ml alcohol) for its analgesic properties, and a poultice of crushed root is applied to swollen joints, as reported by Sato and Nakamura (2019). These three cultures—Chinese, Korean, and Japanese—illustrate the consistent use of Paeonia lactiflora root in infusions, decoctions, tinctures, and poultices across East Asia.

A simple, safe decoction can be made at home: combine 10 g of dried Paeonia lactiflora root with 200 ml of boiling water. Simmer gently for 30 minutes, then strain. Drink one cup (≈200 ml) twice daily. This preparation is traditionally used for menstrual discomfort and mild muscle aches. Caution is advised for pregnant women, as the root may stimulate uterine contractions; it is also best avoided by individuals with bleeding disorders or those taking anticoagulants. As with all herbal remedies, consult a qualified practitioner before use.

The therapeutic effects of Paeonia lactiflora are largely attributed to its well‑studied phytochemicals. Paeoniflorin, a monoterpene glycoside, is the most abundant constituent and is known for its anti‑inflammatory and analgesic actions. Albiflorin, another glycoside, contributes to the root’s calming effect on the nervous system. Paeonol, a phenolic compound, has been shown to inhibit prostaglandin synthesis, supporting the root’s use in menstrual pain relief.
