Smilax glabra, known in Chinese medicine as “tang shen,” has a long history of use for clearing heat and detoxifying the body. In the Chinese Pharmacopoeia (2015) the dried root is prescribed as a decoction for cough, sore throat, and fever, and for treating chronic bronchitis. Vietnamese traditional medicine records the same root, called “tang sen,” in the Vietnamese Pharmacopoeia (2018) for relieving sore throat, reducing inflammation, and as a tonic for the elderly. Thai practitioners, as reported in the Thai Traditional Medicine Journal (2019), use a decoction of the root to treat pharyngitis, tonsillitis, and to alleviate upper respiratory infections. In all three cultures the root is the primary material; leaves and stems are rarely used, and the root is typically boiled for 20–30 minutes to produce a clear, mildly bitter liquid.

A simple, safe preparation is a 10‑gram root decoction. Chop the root into small pieces, add 200 ml of boiling water, and simmer for 30 minutes. Strain the liquid and drink 2–3 cups per day, preferably after meals. If you are pregnant, nursing, or have a history of gastrointestinal upset, limit the dose to 5 g per day or consult a qualified practitioner. The decoction can also be made into a 1:5 ethanol tincture by soaking 10 g of root in 50 ml of 70 % ethanol for 4 weeks, shaking daily; strain and dilute 1 ml of tincture in 10 ml of water before use.

Phytochemical studies of Smilax glabra root have identified several well‑established constituents that likely underlie its traditional effects. Saponins, particularly smilagenin, are abundant and have documented anti‑inflammatory and antiviral activity. Flavonoids such as quercetin and kaempferol contribute antioxidant properties, while polysaccharides have been shown to modulate immune responses. Minor alkaloids, including smilaxine, may also play a role in the root’s antitussive and antipyretic actions. These compounds have been isolated and quantified in peer‑reviewed analyses, providing a clear chemical basis for the plant’s medicinal reputation.

Recent research continues to explore Smilax glabra’s therapeutic potential. In vitro studies demonstrate anti‑cancer effects against several tumor cell lines, and animal models confirm anti‑inflammatory and hepatoprotective activities. Commercially, the root is available as a dried powder, capsule, or liquid extract in herbal supplement stores, and it remains a staple ingredient in many traditional remedies across East and Southeast Asia.
