Santalum album, the fragrant sandalwood tree, has a long record of use in several traditional medicine systems. In Ayurvedic practice, the bark is boiled to make a decoction that is drunk to treat skin eruptions and to relieve itching; the same bark is ground into a paste and applied as a poultice for wounds and burns (Charaka Samhita, 2nd century BCE; Sushruta Samhita, 6th century BCE). The young leaves are steeped in hot water to produce a mild tea that is inhaled or drunk to ease coughs and colds (Charaka Samhita). Traditional Chinese medicine records the bark of S. album as a decoction ingredient for clearing heat and reducing fever, while the leaves are used in a light infusion to calm digestive upset (Shen Nong’s Herbal Classic, 1st century CE; Chinese Pharmacopoeia 2020). Among the Maasai of Kenya, bark fragments are macerated in local spirits and the resulting tincture is applied topically to treat skin infections and to soothe insect bites (Journal of Ethnopharmacology 2015).

A simple, safe tea can be prepared with the leaves. Take 5 g of fresh or dried leaves, place them in a cup, pour 250 ml of boiling water, cover, and let steep for 10 minutes. Strain and sip warm. This infusion is generally well tolerated, but pregnant women should avoid large quantities because sandalwood can stimulate uterine contractions; as with any herbal preparation, use only moderate amounts and discontinue if any skin irritation occurs.

The therapeutic effects of sandalwood are largely attributed to its sesquiterpene constituents, especially α‑santalol and β‑santalol, which have documented anti‑inflammatory and antimicrobial activity. The bark also contains phenolic compounds such as santalol‑derived aldehydes that contribute to its soothing properties on the skin. These compounds provide a biochemical basis for the decoctions and poultices described above.

Today, sandalwood oil is widely commercialized for perfumery, aromatherapy, and cosmetic products, and research continues to explore its anti‑inflammatory and wound‑healing potential. The traditional preparations remain in use in rural communities, and modern studies are validating many of the historical claims made by Ayurveda, TCM, and African ethnomedicine.

Sandalwood essential oil is listed for food use by major flavor regulatory bodies and is employed as a natural flavoring in confectionery, bakery, beverages, and dairy. Usage is limited to flavor character without physiological or therapeutic claims.

Fragrance and cosmetics:
The essential oil is widely used as a key note and fixative in fine fragrances, perfume bases, soaps, detergents, cosmetics, and personal-care aromatherapy products. Commercial wood dust and extracted oil are also used in incense and scented goods where a sandalwood odor is desired.

Properties relevant to use:
Santalum album oil contains 41–55% alpha-santalol, with beta-santalol, (Z)-alpha-santalol, santalols, and sesquiterpenes contributing to its characteristic sweet-woody odor. Typical physicochemical parameters include density 0.965–0.980 at 20°C, optical rotation −18° to −13°, refractive index 1.502–1.509, and acid value ≤5 mg KOH/g; these values support its qualification as a natural aroma compound.

Standards and regulation:
International standards for sandalwood oil are established (e.g., ISO 3518:2002). The species is listed in CITES Appendix II with national controls, and trade documentation is required (e.g., CITES permits and certificates of origin). Flavor and food uses follow major regulatory frameworks for essential oils used as flavoring substances.

Sustainability and sourcing:
Santalum album is assessed as Vulnerable on the IUCN Red List due to wild overharvesting and long growth to maturity. Supply is increasingly sourced from cultivated plantations in India and Indonesia; wild-harvested material must comply with CITES and national regulations, and product documentation typically includes species verification and geographical origin to ensure compliance.
