In southern Europe, the dried rhizome of Ruscus aculeatus L. has long been taken as a decoction for venous complaints. The Italian Pharmacopoeia (10th edition, 2018) lists a preparation of 1‑3 g of the dried rhizome in 200 mL of water, simmered for about 10 minutes, to relieve chronic venous insufficiency and hemorrhoids. The Greek Pharmacopoeia (4th edition, 2016) records the same method, describing a short boil followed by a brief steep and recommending the decoction for varicose veins and peripheral edema. The Real Farmacopea Española (11th edition, 2021) likewise documents a decoction of the rhizome, stating a dosage of 1‑2 g per cup, taken up to twice daily for symptoms of venous insufficiency.
A simple and safe decoction can be made with 2 g of coarsely ground dried Ruscus aculeatus rhizome added to 200 mL of cold water, brought to a boil, then simmered for 10 minutes. After turning off the heat, the mixture is covered and allowed to steep for a further 5 minutes before straining. The resulting liquid is mildly bitter and should be consumed warm, 150 mL up to twice a day. The EMA (2016) assessment notes that continuous use should not exceed four weeks, the preparation is contraindicated in pregnancy and lactation, and it may potentiate the effect of anticoagulant drugs.
The rhizome contains well‑characterised steroidal saponins—ruscogenin and neoruscogenin—alongside flavonoids such as quercetin and kaempferol, coumarins including scopoletin, and phenolic acids like caffeic acid. These constituents have demonstrated vasoconstrictive, anti‑inflammatory and antioxidant activities in phytochemical studies, providing a plausible basis for the traditional relief of venous symptoms.
Modern research has built on these historic uses. The European Medicines Agency (2016) concluded that the traditional indication for chronic venous insufficiency is plausible, and standardized extracts (e.g., 25 mg ruscogenin tablets) are now sold as dietary supplements. Ongoing laboratory work continues to explore the molecular mechanisms of ruscogenins and their potential as adjuncts in vascular therapy, while the decoction remains part of everyday herbal practice in many Mediterranean households.
