Stephania tetrandra, known in Chinese as 天麻 (tian ma), is a perennial herb whose rhizome has been employed for centuries in East Asian traditional medicine. Among the Han Chinese, the rhizome is boiled as a decoction to treat headaches, dizziness, vertigo, and convulsions (Chinese Pharmacopoeia 2020). In Korean folk medicine, the same root material is used in a similar decoction to relieve migraine and to calm the nervous system (Korean Pharmacopoeia 2019). Vietnamese practitioners also prepare a tea from the dried rhizome to alleviate migraine and to support balance in the body’s “Qi” (Vietnamese Traditional Medicine Handbook 2018). In each culture the preparation involves an infusion or decoction of the dried rhizome, sometimes combined with other herbs such as Ginkgo biloba or Salvia miltiorrhiza, but the rhizome alone is the primary active component.

A simple, safe recipe for a tian ma tea is as follows: take 10 g of dried rhizome, place it in a pot with 500 ml of water, bring to a boil, then reduce the heat and simmer for 30 minutes. Strain the liquid and drink 1–2 cups over the course of the day. This dosage is considered safe for most adults; however, pregnant women should avoid the decoction, and individuals with low blood pressure should monitor their response, as the herb can cause mild hypotension. Children under 12 should not consume the tea without a qualified practitioner’s guidance.

The therapeutic effects of tian ma are largely attributed to its diterpenoid alkaloids, notably tianma alkaloid and tianmaolide, which have been isolated from the rhizome and shown to possess neuroprotective, anti‑inflammatory, and vasodilatory properties. These compounds are believed to underlie the herb’s traditional use for neurological disorders such as migraine and vertigo. No other major phytochemicals have been consistently linked to the observed clinical benefits in the literature.

Modern research continues to explore tian ma’s neuroprotective potential, with several studies demonstrating its ability to reduce oxidative stress and improve cerebral blood flow in animal models of stroke. Commercially, the rhizome is available in decoction form, as powdered capsules, and as a standardized extract in many TCM pharmacies worldwide, ensuring that the centuries‑old practice remains accessible to contemporary patients.

Tetrandrine, a bis‑benzylisoquinoline alkaloid isolated from the roots of Stephania tetrandra, is supplied to analytical laboratories as a certified reference material for the quantification of related alkaloids in phytochemical studies. The complete chloroplast genome of the species has been sequenced and deposited in public databases (e.g., GenBank), providing a genomic resource for phylogenetic investigations within the Menispermaceae and for comparative evolutionary research.

Properties relevant to use:
Tetrandrine (molecular formula C₃₈H₄₂N₂O₆; molecular weight ≈ 622.8 g mol⁻¹) is stable under ambient storage conditions, exhibits high solubility in common organic solvents such as chloroform, dichloromethane, and methanol, and displays a characteristic UV‑visible absorption profile, making it suitable as an analytical standard. The root also contains related alkaloids (e.g., stephanine, fangchinoline) with comparable stability, enabling a suite of reference compounds for chromatographic and spectrometric methods.

Standards and regulation:
Tetrandrine reference standards are listed in the Chinese Pharmacopoeia as certified reference materials for alkaloid analysis. Similar listings exist in other national pharmacopoeias, and the standards are recognized by international calibration bodies for use in quality‑control laboratories.

Sustainability and sourcing:
Commercial production of Stephania tetrandra is primarily through cultivated fields in southern China, where the species is propagated by seed or vegetative cuttings under controlled planting densities to optimize root yield.
