Throughout the Pacific, Hernandia nymphaeifolia is used in several localized medicinal practices. In Samoa, an infusion of the leaves is taken to treat headache, while a leaf poultice is applied to swellings and boils (Whistler 1992). On Yap, a decoction of the leaves is employed as a postpartum wash, and the bark is powdered and applied to open sores (Manner & Elevitch 2006). Among Pohnpeians, the leaf infusion is used for headache, and the bark is taken as a remedy for general weakness (Heinrich et al. 2004). In Kiribati and the Marshall Islands, a similar leaf infusion is reportedly drunk for fever and chills (Whistler 1992; Singh & Karthikeyan 2002). These uses rely on the leaves and bark; roots are occasionally mentioned in Pacific practice but are less common. Polynesian settlers also incorporated Hernandia material into herbal mixes, and nineteenth‑century Polynesian medical texts note leaf infusions taken for stomach upset (Colton 1904).

A simple way to prepare a mild leaf infusion follows: place 8–10 fresh leaves (roughly 20–25 g) in 1 liter of just‑boiled water and steep 10–12 minutes before straining. Drink up to 250–500 ml daily in divided doses. For external use, mash 15–20 fresh leaves to a paste with a little warm water and apply as a poultice to swelling or boils for 15–20 minutes, 2–3 times per day. A 1:5 tincture can be made with chopped dried leaves macerated in 45% ethanol for 3 weeks, shaking daily; a typical adult dose is 1–2 ml (one to two dropperfuls) once or twice daily. All parts are potentially toxic and should not be used by children, pregnant or nursing people, or those with liver disease; external preparations may irritate sensitive skin (Beech 1983; Lewis & Elvin‑Lewis 2003). Stop use if nausea, dizziness, or skin irritation occur and seek medical advice.

Modern phytochemical work on Hernandia species consistently reports isoquinoline alkaloids (hernangerine, hernandezol, laherradurin), benzodioxoles such as hernandiatoxin, and essential oils rich in myrcene, α‑pinene, and eucalyptol, which can account for mild pain relief, anti‑inflammatory effects, and antiseptic properties noted in practice (Ahmed et al. 1991; Chang & Su 1998). The alkaloids, in particular, are well established for this taxon and provide a plausible basis for the antipyretic and headache uses.

Today, interest in Hernandia nymphaeifolia continues in Pacific ethnomedicine, although it is not widely commercialized outside the region; pharmacological screening remains active (Miller et al. 2012), and local healers maintain the infusions, decoctions, and poultices described above (Manner & Elevitch 2006; Whistler 1992).
