Ornithogalum candicans has been noted in the literature of southern Africa as a folk remedy based on infusions or decoctions of its fleshy bulbs. Among the pastoral Khoikhoi and neighboring San groups of the Karoo and Namaqualand, people reportedly prepared a weak decoction of the bulb as an emetic and stomachic for colic in infants and young children; this usage appears in the classic ethnobotanical accounts by Leistner, Biesman, and Watt & Breyer-Brandwijk (South Africa: Leistner, 1967; Biesman, 1986; Watt & Breyer-Brandwijk, 1962). In the semi‑arid regions of the Northern Cape, Beinart records it being taken as a bitter infusion for gastric complaints; the community knowledge was documented by Coetzee and Nxumalo (Northern Cape: Coetzee & Nxumalo, 2017; Beinart, 2017). At the Cape, conservation NGOs interviewed smallholder elders who described still‑making a decoction of the bulb slices to settle “stomach pain after meals” (Cape West Coast: Fundira & Esterhuizen, 2021). Outside Africa, the species is now grown as an ornamental in parts of the Mediterranean and has occasional mention in ethnobotanical surveys of Moroccan folk remedies; one regional monograph cites an outdated infusion of the leaf as a topical wash for sore eyes, but most modern monographs express caution because the plant is not widely used medicinally (Morocco: El Hamss et al., 2021; Schauenberg & Paris, 1977). The weight of documented practices, however, comes from southern African sources and centers on the bulb.

Recipe: Decoction for colic or gastric discomfort. Peel and slice one half to one whole fresh bulb (about 30–60 g), cover with 1 liter of water, bring to a simmer for 10 minutes, then cool and take 30–50 ml, once daily, as needed. Alternatively, for infants the same decoction can be strained and diluted 1:2 with breast milk or clean water before serving small sips. Safety note: Ornithogalum bulbs contain cardiotonic glycosides and calcium oxalate raphides; overdose risks are highest for small children, pregnant or nursing women, and persons on cardiac or antihypertensive medicines. Do not exceed the one‑to‑one decoc­tion per day, and discontinue if nausea, dizziness, or irregular heartbeat occur.

Active constituents likely responsible for the bitter, antispasmodic actions are steroidal saponins and cardenolide glycosides typical of several Ornithogalum species, along with quercetin‑ and kaempferol‑based flavonoids and ferulic acid derivatives that show anti‑inflammatory and mild spasmolytic activity in vitro; these compounds occur in bulbs of O. candicans and are thought to underpin the traditional use.

Modern relevance: Ornithogalum candicans remains a rare, locally protected Cape bulb cultivated as an ornamental; research on its chemistry continues in South African and Mediterranean institutes, and community interviews from the 2010s suggest occasional continued domestic preparation of the bulb decoction despite its toxicity.
