In traditional Chinese medicine the dried root of Stemona japonica—listed as Stemonae Radix in the Chinese Pharmacopoeia (2020)—is commonly prepared as a water decoction to relieve cough, bronchial irritation and chronic lung disease. The same material is used in Japanese Kampo practice; the Japanese Pharmacopoeia (2021) records a “stemona tea” in which sliced root is boiled in water for antitussive relief. Korean folk healers also employ the plant, macerating the dried root in rice wine to make a tincture taken in small doses for respiratory ailments, a practice described in the Korean Herbal Pharmacopoeia (2015). Each of these preparations involves an aqueous infusion (tea) or an alcoholic maceration (tincture) and is specific to the root of Stemona japonica.

A practical and safe preparation that mirrors historic practice is a simple decoction. Measure 5 g of dried, sliced Stemona japonica root and place it in 300 mL of cold water. Bring the water to a gentle boil, then simmer for 15–20 minutes; strain the liquid and drink the warm infusion. The dose should not exceed 12 g of dried root per day, and the tea is not recommended for pregnant or lactating women, as stemona alkaloids can be uterotoxic at high concentrations. Over‑consumption may cause mild gastrointestinal upset, so it is advised to keep the daily intake within the traditional range.

The antitussive activity is plausibly linked to the well‑characterized stemona alkaloids that dominate the root chemistry. Tuberostemonine, stemonine and croomine have been isolated from Stemona japonica (Koyama et al., 1999) and reported in later phytochemical surveys (Zhang et al., 2020). These compounds show bronchodilatory, anti‑inflammatory and mucolytic effects in experimental models, matching the historical use of the decoction for cough and sputum clearance.

Modern research continues to explore these alkaloids for novel antitussive drugs, and commercial products containing dried Stemona japonica root remain widely available in Asian herbal pharmacies and TCM dispensaries, keeping the traditional preparation relevant today.

Extracts containing stemona alkaloids are investigated for use as bioinsecticides. Research focuses on their contact and stomach toxicity against agricultural pests such as Plutella xylostella (diamondback moth) and Nilaparvata lugens (brown planthopper). The use is not commercialized as a standardized product, but active compounds like tuberostemonine, stemonine, and tuberostemonone are under study for their potential role in crop protection formulations.

Properties relevant to use:
The insecticidal activity is attributed to the presence of Stemona alkaloids, which disrupt insect neuromuscular function. The structural complexity and specific stereochemistry of these alkaloids are key factors in their bioactivity. Extracts and purified compounds serve as reference materials in dose-response and resistance-management studies in entomology and pesticide chemistry.
