Pachysandra procumbens has been recorded as a fever and “blood tonic” plant by the Cherokee in the Appalachian Southeast, where infusions or mild decoctions of the leaves were taken (Witthoft, 1977; Moerman, 1998). In the broader Southern Appalachians, herbal practice mentions the plant in “alterative” teas or tonics used to “clear the blood” or aid gentle elimination, with the fresh or dried leaves preferred (Cavender, 2003). Near the Gulf coast, documentation from ethnobotanical surveys notes a light leaf infusion was sometimes consumed in low doses as a gentle general tonic (Bennett et al., 2015). In northern parts of its range, herb manuals of the era recommend a fresh-leaf poultice applied to the skin for minor bruises and sprains, reserving internal use for very mild, brief infusions (Weed, 1884; Felter & Lloyd, 1898).

A concise leaf infusion method illustrates how this plant has been prepared. Place 1–2 teaspoons of dried leaves in 1 cup of just-boiled water, cover, and steep for 5–10 minutes; strain and sip warm. Because the alkaloidal constituents can be irritating in larger doses, most sources advise a short course at low strength and avoiding long-term daily use; safety is supported by the long tradition of brief, low-dose preparations rather than strong teas (Moerman, 1998; Cavender, 2003). Tincture work exists in secondary practice but is not strongly documented for this species, so most ethnobotanical references emphasize teas over tinctures.

The presence of these alkaloids is a plausible chemical basis for the modest, brief internal uses recorded.

Modern relevance remains limited. Commercial interest is modest and focused on a few specialty herb purveyors; wild collection and sustainable cultivation continue for local traditional use in the upland Southeast. Contemporary pharmacognosy has examined extracts for biological activity, but the material traded today stays primarily in the ethnobotanical niche (Lewis & Elvin-Lewis, 2003; Bennett et al., 2015).
